If you have decided that you would like for your child to be booked in with us, the best and quickest way to do this is to fill in the below questionnaire and send it back to us by email (info@agnesali.co.uk).
We will then get in touch with you with available dates either by phone or email. Once a date is pencilled in, an invoice for a deposit will be raised, and when payment is received the appointment confirmation will be emailed out to you.
If you are not sure whether our services are the right thing for your child, please contact us through the contact tab on this website, so we can arrange a no obligation phone call. You can also look at page 3-4 of below questionnaire, and if you tick ‚always‘ and ‚often‘ on several of the symptoms listed, we are likely to be able to have a positive impact on your child‘s development, where if you tick ‚never‘ and ‚occasionally‘ only, we may not be the right people to help.
We are currently booking about 3-4 months ahead for new assessments.
